Every virtual card you issue counts once, whether it is charged or not. A single-use card that is never used still counts, because it was issued.
Not on the Developer and Production plans. Your agents pay the merchant price, and the plan covers the cards they use to pay it.
Issuing pauses at the monthly limit and your agents get a clear error they can handle. You can move up a plan from the dashboard at any time.
